|
It was reported that after repair of the mayfield infinity skull clamp (a1114), the customer reused the mayfield for a surgery.Before the surgery, the two arms did not slide, not allowing the patient's skull to be fixed for the surgery.The clinician took a video of the malfunction which was sent to our service and repair site.The surgeon continued the surgery after 90 minutes delay due to the switching to another mayfield they had in stock.No injury to the patient was observed.The exact event date is unknown.

The mayfield infinity skull clamp (a1114) was returned for evaluation: device history record (dhr) - the dhr was reviewed and shows no abnormalities related to the reported failure.Failure analysis - the investigation of the returned device found no device deficiencies that would have contributed to the reported complaint.The video sent to us by the customer was very helpful, but we could not replicate the issue while testing the unit according to the manufacturer's specifications.No maintenance is required at this time.However, the unit subjected to a successful functional check and will be marked with an instance number.Root cause - the complaint is not confirmed.The returned unit is in good working order and meets all manufacturer¿s specifications, and no maintenance is required.Probable root cause is improper use.No further investigation required based on the acceptability of risk and no adverse trends identified.This will be monitored and trended going forward.At present, we consider this complaint to be closed.
