A physician was attempting to use a hawkone device for treatment of a calcified plaque lesion in the with 70% stenosis in the superficial femoral artery.The vessel was not pre-dilated.The ifu was followed.It was reported the thumb switch would not go all the way up to pack the plaque.The device would turn off but not go all the way forward.Device appeared full so cleaned device, not much plaque removed.Reinserted device and again thumb switch would not go all the way forward to pack plaque.Device removed.Used 6-0 pta.Removed filter wire (6-0) and filter had plaque in device.No patient injury was reported.When the device was returned, it was noted that the device was damaged.
