Event Description
The customer reported to olympus, the brightness of the light source could not be controlled automatically.The issue occurred during middle of the colonoscopy procedure, and the entire endoscopy cart was being utilized during this procedure.The automatic brightness on the light source was the issues.The customer was able use the manual adjustment to complete the procedure using the same device without delay or error messages.There was no patient harm associated with the event.
 
Manufacturer Narrative
The device was returned to olympus for evaluation and the customer's allegation was confirmed; auto brightness was not working and image was dull.The device evaluation found a faulty scope socket and evidence of a third party repair.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the legal manufacturer's final investigation.Additional information: h6.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over eight (8) years since the subject device was manufactured.A definitive root cause was not identified, however based on the results of the investigation, the probable cause of the malfunction would likely be automatic dimming does not work and this occurred due to scope socket failure.Olympus will continue to monitor field performance for this device.
