Event Description
|
On (b)(6) 2023 physician used a venaseal closure system during a bilateral great saphenous vein (gsv) ablation and phlebectomies pro cedure.The lumen was flushed prior to use.The ifu (instruction for use) was followed during preparation, procedure, post-procedure.A guidewire was used for the insertion of the catheter.Tumescent infiltration was not utilized.General anesthesia was used.Transducer compression was used.Procedure was completed as per ifu.The sapheno-femoral junction (sfj) was confirmed as clear of glue by surgeon at completion of procedure with full gsv closure bilaterally.Both legs were wrapped with uniform compression bandage as per protocol and left intact for 3 days.Bandages removed by patient at day 3 as per surgeon instructions and soon after complained of shortness of breath and presented to local emergency department.Subsequent investigations revealed bilateral tongues of thrombus from both left and right common femoral veins.Patient was transferred to tertiary centre for management of pulmonary embolism.
