Event Description
The customer reported it was difficult to see the ultrasound images using the ultrasonic probe.They adjusted the settings and used it to complete the unspecified procedure.The device was returned for evaluation.During the device evaluation, leakage of the ultrasound media was found.There were no reports of patient harm.This medical device report (mdr) is being submitted to capture the malfunction found during evaluation.

E1: establishment name: (b)(6).The device was returned to olympus for evaluation and the customer's allegation was confirmed.In addition to the findings reported in b5, the device evaluation found there was a hole, scratches and indentations on the tip sheath.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the event was unable to be identified.The presence of holes in the tip sheath indicates that some stress was applied to the tip sheath.As the internal ultrasound medium leaked out from the hole and decreased, ultrasound could not be transmitted normally and ultrasound images could not be visualized properly, leading to the phenomenon that the ultrasound images were difficult to see.Olympus will continue to monitor field performance for this device.
