Event Description
The customer reported to olympus that during preparation for use, the light source narrow band imaging and auto-adjustment were not working, and the light was too bright without the dimming operation on the xenon light source.The customer was unable to operate light button.The processor was just repaired and sent back, and this was the first time it was used.There were no reports of patient injury or medical intervention associated with this event.

The device was returned and evaluated, but the allegation was not confirmed.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.   new information added to the following field: h6.Correction on field h10, as the findings of the evaluation were inadvertently missing in the initial medwatch.Since the device is more than 15 years old, the device history record was unable to be reviewed.However, olympus only releases products to market that meet all manufacturing specifications and final product release criteria.In addition, the following non-reportable malfunctions were found during the device evaluation: -narrow band imaging did not work.Based on the results of the investigation, a definitive root cause could not be identified.Olympus will continue to monitor field performance for this device. .
