Event Description
It was reported that the twist clamp of the minicap extended pd transfer set was unable to close; further described as ¿was unable to close tightly¿.This was observed during use of the device for peritoneal dialysis therapy.There was no report of patient injury or medical intervention associated with this event.No additional information is available.
 
Manufacturer Narrative
H10: the sample was received for evaluation.A visual inspection with the naked eye noted a broken occluder foot on the main body of the twist clamp.Functional tests which included leak, clear passage, and clamp function tests were performed; leak testing and clamp function testing identified a fluid flow with the twist clamp in closed position.Torque engagement testing was performed and the engage and disengage force to open and close the twist sleeve was acceptable and met specification.The reported condition of unable to close the twist clamp was not verified; however, the sample did not meet specification for leak- fluid flow with clamp closed.The broken occluder foot was the cause of the leak.The cause of the condition could not be determined; however, if the device was exposed to chemical agents such as hydrogen peroxide, alcohol or bleach, as listed on product packaging, this could damage the transfer set materials.Should additional relevant information become available, a supplemental report will be submitted.
