Event Description
The gore field sales associate reported the following: on (b)(6) 2023, the patient underwent endovascular treatment of a zone 9 abdominal aortic aneurysm using gore® excluder® conformable aaa endoprostheses.It was reported that during the procedure when attempting to retract the delivery catheter of the gore® excluder® conformable aaa endoprosthesis through the gore® dryseal flex introducer sheath some resistance was felt.The physician used some force and was able to retract the delivery catheter.The leading olive had become separated and ended up in the aneurysm sac and behind the implanted device.The physician chose to leave the olive as ¿endo trash¿ since it was not going to move.The patient tolerated the procedure.

Per gore engineering the reported failure mode of leading olive separation could not be independently confirmed.No images, nor the physical device were available to allow for evaluation of the reported failure mode.The physician reported some elevated force during withdrawal, but the root cause of the leading olive separation cannot be established with the available information.
