Event Description
It was reported that during implant the leadless implantable pulse generator (ipg) exhibited a failed deployment with deflection on and poor electricals were obtained.On repositioning, the deflection button was noted to be broken and very loose and was not responding appropriately.The leadless ipg was attempted/not used and replaced. no patient complications have been reported as a result of this event.

Manufacturer Narrative
