Event Description
|
Information was received from health care provider (hcp) via a company representative (rep) regarding a patient receiving intrathecal hydromorphone (concentration and dose unknown) via an implanted pump for non-malignant pain.It was reported the physician assistant (pa) was having issues connecting the tablet to the pump.It was noted they had 6 or 7 refills today and he had not had any problems connecting to those pumps.Technical services had the rep try to interrogate the pump with the communicator and the tablet.It was confirmed that he was able to see the port of the pump and confirmed that there was no electromagnetic interference (emi) in the room.The issue was not resolved through troubleshooting.The pa was going to try troubleshooting with trying another tablet, change positions and check orientation of the pump.The issue was not resolved, and he would call back to let us know if it was resolved or not.Patient symptoms were not reported.

Additional information received from a company representative (rep) indicated that there was troubleshoot/diagnostics performed that was unsuccessful.The cause ofthe event was not determined.Actions/interventions taken included the healthcare provider (hcp) used another tablet.Other actions that were taken, or are planned, to resolve the event included the application was reinstalled.

Additional information received from the manufacturer representative (rep) indicated that the hcp was instructed to reinstall the synchromed application in attempt to resolve connection issue and to call the rep if additional assistance was needed with the issue.No further information was received from hcp.Since event occurred, the hcp had left clinic and no further information was able to begathered from him.The patient's weight and cause of the connection issue was unknown and it was unknown if reinstalling the synchromed application resolved the issue.

Additional information was from a company representative (rep) stated that the patient came in today for surgical consult to get pump explanted and he was unable to read the patient's pump.However, the patient had too much electromagnetic interference (emi) exposure and thus he could not read the pump which he had never encountered before.
