The device was returned to olympus for evaluation and the customer's issue was confirmed causing no visible image due to the printed circuit board.The evaluation found corrosion on the cables and scratches on the top cover.The investigation is ongoing and follow up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported to olympus, the evis exera ii video system center experienced a pinkish image which made it difficult to observe the tissue.The issue was found during preparation for a diagnostic endoscopy.The procedure was completed with a similar device.There were no reports of patient harm.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.New information added to the following fields: h6.Corrected fields: d10 and correction on field h10 (non-reportable finding was omitted from the initial medwatch).A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 14 years since the subject device was manufactured.Based on the results of the investigation, a definitive root cause could not be determined.However, it is like that the event reported as "pink display appeared" was caused by circuit printed board failure.In addition, the following non-reportable malfunctions were found during the device evaluation: corrosion was found in the cable.Olympus will continue to monitor field performance for this device.
