It was reported that two months post implant, the right ventricular (rv) lead displayed oversensing and a lead integrity alert (lia) was triggered for sensing integrity counter (sic) and pacing impedance variation. There was a concern for lead dislodgement due to a rise in pacing impedance, drop in r-waves, and rise in capture thresholds.A lead revision was done the next day. When the implantable cardioverter defibrillator (icd) pocket was opened, it was discovered that the icd had been rotated/twiddled multiple times causing the lead to pull back. The lead was explanted and replaced, and the icd was reimplanted. No further patient complications have been reported as a result of this event.
