The device was returned to olympus for evaluation, and the evaluation found additional non-reportable findings were as follows: leak at biopsy port, bending section cover (a-rubber) was peeling on both sides, the image was intermittently flickering when moving the bending section, smashed/dent on the insertion tube, scratches on light guide tube (lg tube), scratches on video cable, peeling labels on scope connector, dents and scratches on video connector.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.

The customer reported to olympus, the hysterovideoscope biopsy port spun, making it difficult to remove the stop cock when attached.The issue was found during reprocessing.There was no delay to the procedure and the procedure was completed.There were no reports of patient involvement.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 8 years since the subject device was manufactured.Based on the results of the investigation, and the information provided, the cause of the loose biopsy port could not be determined.Olympus will continue to monitor field performance for this device.
