The device was not returned to olympus, and it is not expected to be returned for evaluation of the reported issue.The investigation is ongoing, and a supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
It was reported that the endoscope reprocessor was reprocessed with a non-applicable cleaning tube.Additionally, it was found that the cyf-va2/cyf-vha (visera cysto-nephro videoscope/cysto-nephro videoscope), which was reprocessed from the reported product, was being used by patients.There was no report of patient harm.This report is linked to the patient identifiers (b)(6).
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, a definitive root cause of the report of the device being reprocessed with an incompatible/unsupported connecting tube could not be determined, however, the issue was likely the result of the user not carefully reading the instruction manual and implementing the scope reprocessing without a full understanding of the instructions on the list of compatible endoscopes/connecting tubes.The event can be detected/prevented by following the instructions for use section below: -oer-6 cleaning tube application table.Olympus will continue to monitor field performance for this device.
