The device was returned to olympus for evaluation, and the evaluation found that there was a loose tip for advanced diagnostics, 16a no ke45 at the forceps cover, bending section cover or distal sheath's glue had cracks, there were multiple broken elements, the control body had a cust sticker, and do not repair advanced diagnostics grip/scope cover dry.The investigation is ongoing.A supplemental report will be submitted upon completion of the investigation or when additional information becomes available.
 
Event Description
The customer reported to olympus that while using the gastrovideoscope at the beginning of the endoscopic ultrasound procedure(eus), there was a compromised image and they were unable to see the whole ultrasound image causing the procedure to be extended by five minutes.The procedure was completed with another eus device and no patient harm was reported.There is no evidence that it was a life threatening the delay was short and no additional intervention or hospitalization was needed to preclude permanent.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, a root cause of the reported patrial ultrasound image issue could not be determined, as the issue could not be reproduced during the device evaluation.Additionally, the lack of ke45 adhesive at the forceps cover could not be determined.Olympus will continue to monitor field performance for this device.
