The device was returned to olympus for evaluation, and the customers allegation was confirmed.The evaluation found the following reportable issues: the non-olympus outer tube and laser markings where on the light guide post with a missing direction pin.The following non-reportable malfunctions were found during the device evaluation: the fiber was broken, there were dents on the frame, there was debris under the eyepiece window with minor scratches on the funnel, the optical fiber system had a foggy image.The investigation is ongoing and a supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported to olympus that the telescope, 30°, 4 mm has a piece missing in the arbitrator hole.The issue was found during preparation for use.There were no reports of patient harm.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.Corrected fields: d8.Additional information added to field h3, h6.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 10 years since the subject device was manufactured.Based on the results of the investigation, the cause of this malfunction was identified to be third party repair of the device.Three attempts were performed to obtain additional information, but no response was received from the customer.Olympus will continue to monitor field performance for this device.
