Event Description
It was reported that during a cryo ablation procedure, the mapping catheter was stuck in the right superior pulmonary vein (rspv) and the end of the mapping catheter was cut by thoracotomy.After a portion of the pulmonary vein (pv) remained ligated to the mapping catheter. the case was completed with cryo. no further patient complications have been reported as a result of this event.
 
Event Description
Additional information was received indicating that fragments of the device remain in the patient's body.It was not possible to remove the entire device during thoracotomy.No further patient complications have been reported as a result of this event.
