Event Description
|
According to the reporter, during a laparoscopic inguinal hernia repair procedure, when the device was positioned to be fired, the clip did not form, and just fell out of the end of the device.The surgeon attempted to use it again, the device was fired but no clips appeared on the end of the device.The surgeon double tapped and a clip fell out.The next firing appeared normal however when the device was positioned and fired the clip ¿shot¿ off the end like a projectile.Another device was used to resolve the issue.There was no patient injury.
