Event Description
According to the reporter, during a laparoscopic colectomy, when clamping the tissue, the jaw of the device did not clamp properly, moreover there was an uneven tissue cut, sometimes the device cuts, sometimes it did not.There was no problem with the hemostasis.There was no patient injury.A photo was received showing that the active waveguide of the device had chipped.

According to the reporter, during a laparoscopic colectomy, when clamping the tissue, the jaw of the device did not clamp properly, moreover there was an uneven tissue cut, sometimes the device cuts, sometimes it did not.There was no problem with the hemostasis.Replaced with another device and it worked fine.A photo was received showing that the active waveguide of the device had was dented.
