The issue was troubleshooted via email support.The customer was advised to ensure the light source unit was in auto mode for brightness, check the light source cable to ensure secure on both ends and not damaged, possibly try another cable, and to check lamp hours.The customer stated that the light source and processor were looked at and there was not a cord connecting the two units.The customer was advised to connect the light source cable on the video system center and the light source.The issue was resolved.The device was not returned to olympus for evaluation.The investigation is ongoing and follow up with the user facility is currently being performed.A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Event Description
The customer reported to olympus, the xenon light source brightness adjustment did not work.It was unspecified when the issue was found.There were no reports of patient harm.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.New information added to the following fields: h4, h6.Corrected field: d10.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over ten (10) years since the subject device was manufactured.Based on the results of the investigation, a definitive root cause could not be determined.Olympus will continue to monitor field performance for this device.
