Event Description
The customer reported to olympus, the evis lucera duodenovideoscope channel tube had resistance.Forceps insertion problem.The issue was found during reprocessing.There were no reports of patient or user harm associated with this event.
 
Manufacturer Narrative
The device was returned to olympus for evaluation.The customer¿s allegation of forceps insertion problem was not confirmed.The evaluation found the forceps elevator wire was broken.Additionally, due to pinhole on the channel tube, water tightness was lost.The scope cover case and light guide lens had a chip.The root of the insertion tube was wrinkled.The objective lens was slightly scratched and chipped.The switches number 1 and 4 were damaged.The investigation is ongoing [and follow up with the user facility is currently being performed].A supplemental report will be submitted upon completion of the investigation or if any additional information is provided by the user facility.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the root cause of the blockage in the channel was the k-wire (knob wire/forceps elevator wire) broke by fatigue fracture caused by repeated manipulation of forceps elevator.Strands raised by brushing around the forceps elevator or by being caught on a distal cover when the cover is attached/detached.Olympus will continue to monitor field performance for this device.
