Event Description
|
According to the reporter, during a laparoscopic cholecystectomy procedure, the clip applier locked on to the cystic artery and was unable to be released or removed.To resolve the issue, many attempts was made to remove the clip applier, and it was released and was able to be removed.There was no patient injury.

Event Description
|
According to the reporter, during a laparoscopic cholecystectomy procedure, the clip applier locked on to the cystic artery and was unable to be released or removed.To resolve the issue, the surgeon had to cut the clip applier off with tissue in the jaws from the cystic artery.
