According to the reporter, during an open surgery, when firing, the clips did not come out.Another device was used to resolve the issue.There was no patient injury.

Event Description
|
According to the reporter, during an open surgery, one clip was fired, the surgeon was able to squeeze the handle, but the clips were not loading properly into the jaws.One clip fell into the patient's cavity and was able to be removed with a grasper.It was noted that there was increased bleeding but less than 500cc, slower hemostasis, and an increased duration of surgery but not more than 30 minutes.Another device was used to resolve the issue.
