It was reported that the patient with this cardiac resynchronization therapy defibrillator (crt-d) received one inappropriate shock and anti-tachycardia pacing (atp) for atrial fibrillation (af) with rapid ventricular response (rvr).Technical services (ts) provided programming recommendations.No additional adverse patient effects were reported.It was noted that the device was reprogrammed, patient's medication was adjusted, and additional follow-up and lab work performed with cardiologist.Later, over the course of 3 days, this device delivered 62 inappropriate electric shocks due to uncorrelated template programming.Therapy was exhausted during these episodes.Additional reprogramming was performed.Currently this device remains in service.
