Event Description
A physician reported that during an intra-ocular lens (iol) implant procedure, the lens was gradually and smoothly injected into the eye with a 2.2 corneal incision with the tip inside the corneal wound.At the end part of injecting the iol the cartridge snapped and its tip broke off inside the eye along with the iol inside the tip.There was no resistance felt while injecting the iol.The iol was immediately taken out as it was in its folded state.Then the corneal wound was extended with a 3.2mm keratome and the tip was removed with forceps under cohesive viscoelastic.The iol was then reloaded in another company cartridge and then reinjected in the eye.Additional information has been requested.

The product was not returned.Product history records were reviewed and documentation indicated the product met release criteria.The lens model/diopter, handpiece and viscoelastic used weren't provided.It is unknown if qualified products were used.The product investigation could not identify a root cause for the reported complaint.A root cause cannot be determined without physical evaluation of the sample.The instructions for use (ifu) instructs: the company intraocular lens (iol) delivery system is for implantation of qualified company foldable iols.No unqualified lenses should be used with the company iol delivery system.The company cartridges are qualified for use with compatible company handpieces for the surgical implantation of company qualified foldable iols.Company foldable iols are qualified for use with a company qualified delivery system (handpiece and cartridge) and ophthalmic viscosurgical device (ovd) combination.The use of an unqualified combination may cause damage to the iol and potential complications during the implantation process.File will be reopened if the sample is received.The manufacturer internal reference number is: (b)(4).
