Event Description
It was reported that a minicap transfer set had a fractured valve (twist clamp) and it did not close properly which resulted in a leak.This was observed during use of the device for peritoneal dialysis therapy.There was no report of patient injury or medical intervention associated with this event.No additional information is available.
 
Manufacturer Narrative
B5: the patient was treated with 1 g of vancomycin and 100 mg of amikacin ip (intraperitoneally).No disinfectant products were used on the transfer set.The transfer set was replaced.B7: catheter implanted in september 2023.H10: the actual device was not available; however five (5) photographs of the sample were provided for evaluation.The photographs were visually inspected and a broken occluder leg beneath the twist clamp was observed causing a fluid flow through the set with the twist clamp closed.The reported condition was verified.The cause of the condition could not be determined; however, exposure to chemical such as foreign solvents, dyes, or cleaning agents that damage the transfer set materials.A batch review was conducted and there were no deviations found related to this condition during the manufacture of this lot.Should additional relevant information become available, a supplemental report will be submitted.
