Event Description
Reportable based on device analysis completed on 4jan2024.It was reported that visualization issues occurred.The opticross hd imaging catheter was selected for ultrasound examination of the target lesion.During preparation, the screen was black and no image was shown.The procedure was completed with another of the same device.There were no patient complications reported and the patient condition following the procedure was stable.However, device analysis revealed the imaging window was twisted and the tip was stuck with the guidewire.
 
Manufacturer Narrative
The device was returned for analysis.Visual inspection revealed the sheath and imaging window were kinked, the imaging window and imaging core were twisted, and the tip was stuck with the floppy part of the guidewire.Microscopic inspection revealed the guidewire exit port and tip were in good condition.Impedance testing showed an electrical open at the proximal end of the catheter between 140-150 cm from the distal housing which is related to breakage of the coaxial cable.Functional testing could not be performed due to the condition in which the device was returned.
