It was reported that this was an arteriotomy closure of the left common femoral artery using a prostyle after a percutaneous transluminal angioplasty interventional procedure using a 5f sheath.Reportedly, after noticing pulsatile flow, it was attempted to lift the lever (step 1), however, the lever didn¿t move at all even after several attempts.Manual compression was used to achieve hemostasis.There was no adverse patient sequela and no reported clinically significant delay in the procedure or therapy.No additional information was provided.

Analysis was performed on the returned device.The reported mechanical jam foot/lever was not confirmed as the lever was opened and closed with no difficulties or anomalies noted.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no similar incidents from this lot.The investigation was unable to determine a conclusive cause for the reported difficulties.Factors that may contribute to difficult to deploy the foot include, but are not limited to; tissue or suture caught in the foot, or posterior cuff partly deployed in the foot.The treatment appears to be related to circumstances of the procedure.Based on the information reviewed, there is no indication of a product quality issue with respect to manufacture, design or labeling.
