It was reported that the patient is seeing oor messages 1098 and 1703 today.Agent reviewed reasons for this message and advised that the caller have patient meet with managing healthcare provider (hcp).The caller noted that the patient had a revision of their extension related to the oor messages in the past.No symptoms were reported.Refer to pe706026260 for the previous oor event and extension revision.

Additional information received from the manufacturer¿s representative (rep), which was confirmed with the healthcare provider (hcp), reported they saw the patient on january 29th.X-rays were performed which showed a kink, possibly a break, in the right extension wire.The patient was scheduled for a revision on (b)(6) 2024.

|
It was reported patient (pt) no longer receiving effective stimulation due to multiple high impedances.After multiple impedance checks in clinic, patient was brought into the operating room.The lead impedances were within range, but the extension impedances were all out of range.The healthcare provider (hcp) tested the extension and the lead.After replacing the extension all impedances were within range.The issue was resolved.
