A return material authorization (rma) was issued to the customer requesting to have the intuitive device returned.However, isi has not received the product involved with the alleged issue to perform failure analysis.Additional information is being gathered to determine the contribution of the device to the customer reported issue.A follow-up mdr will be submitted if the product is returned (post failure analysis evaluation).

Event Description
|
It was reported that during a da vinci-assisted liver resection surgical procedure, the insulation sheath melted.The procedure was completed with no reported injury.Intuitive surgical inc.(isi) contacted the site and obtained the following additional information regarding this event: the instrument was inspected prior to use and there was no damage or anything out of the ordinary.The thermal damage was first observed intraoperatively during use.According to the surgeon, the thermal damage to the instrument was probably caused by too high energy effect.The instrument did not collide with an energy instrument during the procedure and there was no arcing observed during the procedure.

Intuitive surgical, inc.(isi) did receive a da vinci product to perform failure analysis.The maryland bipolar forceps was analyzed.The instrument exhibited charring and localized melting between the grips.This failure is caused in most cases by insulation degradation and charred tissue forming a conductive path.The device has passed the electrical continuity test.The complaint was confirmed by failure analysis.
