It was reported that the atrial tachycardia/atrial fibrillation (at/af) episodes were not counted.The cardiac resynchronization therapy defibrillator (crt-d) remains in use.No patient complications have been reported as a result of this event.

Additional information received indicated, clinic confusion due to accounting of monitored at/af episodes.Upon review of device data, it was noted that indeed that it is likely that there is increase of the at/af episode counter in previous devices and it has been discovered a deviation in the behavior of this crt-d platform that appears to be related to the performance of the programmer.
