It was reported that on 19-january-2024 during sterile processing, the instruments in question were found to be broken.Both products have broken locking mechanisms in the handle.There was no reported patient or procedure involvement.No further information is available.This report is for a t-handle with quick coupling.This is report 2 of 2 for (b)(6).

Depuy synthes is submitting this report pursuant to the provisions of 21 cfr, part 803.This report may be based on information which depuy synthes has not been able to investigate or verify prior to the required reporting date.This report does not reflect a conclusion by fda, depuy synthes or its employees that the report constitutes an admission that the device, depuy synthes, or its employees caused or contributed to the potential event described in this report.H3,h4,h6 the product was not returned to depuy synthes, however photos were provided for review.The photo investigation revealed that 311.44, t-handle with quick coupling the provided evidence was not sufficient to confirm the reported event.Since the device was not returned, a dimensional inspection cannot be performed.The overall complaint was not confirmed as the observed condition of the 311.44, t-handle with quick coupling would not contribute to the complained device issue.Based on the investigation findings, it has been determined that no corrective and/or preventative action is proposed.There is no indication that a design or manufacturing issue has caused the complaint condition.As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.H4, h6 part:311.44, synthes lot: 4553810, supplier lot: n/a, release to warehouse date: feb 27,2003, manufactured by: synthes brandywine, no non conformance reports were generated during production.Device was used for treatment, not diagnosis.If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.

Depuy synthes is submitting this report pursuant to the provisions of 21 cfr, part 803.This report may be based on information which depuy synthes has not been able to investigate or verify prior to the required reporting date.This report does not reflect a conclusion by fda, depuy synthes or its employees that the report constitutes an admission that the device, depuy synthes, or its employees caused or contributed to the potential event described in this report.The product was returned to depuy synthes for evaluation.The depuy synthes team conducted a visual inspection of the returned device.Visual analysis of the returned sample revealed that had the tap holder assembly (locking mechanism) not working as intended, looks like the inner mechanism is broken.The allegation can be confirmed.A dimensional inspection was unable to be performed due to post manufacturing damage.A functional test was performed and the inner mechanism its damage.The device fail the test.The overall complaint was confirmed as the observed condition of the t-handle with quick coupling would contribute to the complained device issue.Based on the investigation findings, and it has been determined that no corrective and/or preventative action is proposed.There is no indication that a design or manufacturing issue has caused the reported complaint condition.As part of depuy synthes quality process, all devices are manufactured, inspected, and released to approved specifications.Additional monitoring for any potential safety signals will be conducted through complaint trending and other post-market safety surveillance activities.H3, h6: device history record (dhr) review conducted: part: 311.44, synthes lot: 4553810, supplier lot: n/a, release to warehouse date: feb 27,2003, manufactured by: synthes brandywine, no ncrs were generated during production.Review of the device history record(s) showed that there were no issues during the manufacture of this product, and any sub-components, which would contribute to this complaint condition.Device was used for treatment, not diagnosis.If information is obtained that was not available for the initial medwatch, a follow-up medwatch will be filed as appropriate.
