Event Description

Literature was reviewed regarding gastrointestinal bleeding (gib) in ventricular assist device (vad) patients.Gib was defined as the presence of hematemesis, hematochezia, melena, and acute anemia with a positive fecal occult blood test.Bleeding was identified via endoscopy, esophagogastroduodenoscopy (egd), push enteroscopy, or colonoscopy.The authors described patient deaths which were categorized as all-cause mortality.The causes of death were gib, ischemic stroke, and other unknown causes.There were patients who experienced gib wither within thirty days of implant, after thirty days, or after one year post implant.Treatment consisted of argon plasma coagulation or blood transfusions with a median of three units of packed red blood cells (prbc).Arteriovenous malformations were the predominant etiology.Many of the patients also experienced acute kidney injury.The status of the devices is unknown.No additional adverse patient effects were reported.
