A facility reported a codman perforator (261221) broke into pieces when performing left craniotomy posterior fossa.There were two sets of drills used with the perforator.The first drill was electric stryker pi drive, however it was changed to a medtronic pneumatic midas drill since it was not working.The perforator clicked in place in the drill, and only one burr hole was created.The parts did not fall into the surgical site.The items were removed from the sterile field and the staff made sure that all of the fragments and pieces were there.An x-ray was not taken as it did not fall into the surgical site.There was no delay in delay in the procedure due to product problem, and there was no harm to the patient.
