Event Description
It was reported that manufacturer representative (rep) was brought into the cath lab because patient (pt) had a cardioversion procedure today where the implantable neurostimulator (ins) was only turned off, it was not programmed per the guidance in the ifp.The caller indicated that after the cardioversion, an attempt was made to connect to the ins via the pt's handset and it would not connect, would only get to '74%' on the communication 'spinning' screen.Caller stated he then removed the links from the handset and then called pats.Caller attempted to connect to ins with tablet and was unable to do so.Agent advised caller could try 'reset neurostimulators' on dbs app and caller did this.After resetting, caller attempted to connect via tablet and saw message 'invalid active group-no active group.No active group defined on the device, would you like to activate' the caller attempted to activate and after this still could not connect to ins.Caller attempted connecting again and noted getting an option to search for previous device which he did without success--the ins did not allow communication.The caller made one more attempt to connect to ins with pt's handset and could not do so.Agent noted this would be documented and escalated and there would be follow up with rep on potential next steps.No symptoms reported.

Event Description
Additional information received from the manufacturer¿s representative (rep) reported they assumed the damage was due to the cardioversion.Multiple attempts were made to connect with the device, with no success.The device was going to be replaced on (b)(6).

Event Description
Additional information received from the manufacturer¿s representative (rep) reported prior to the patient undergoing cardioversion stimulation was turned off by a fellow.The cardioversion took place on (b)(6) 2024 with a prior cardioversion taking place on (b)(6) 2020 at 200 joules (before they had dbs implanted).The energy used during the cardioversion was 200 joules by maker zoll.During cardioversion the paddles were placed in the anterior left chest and posterior mid back with the neurostimulator being implanted in the right chest.
