The field service representative (fsr) checked for over occlusion and cleaned the optical sensor but the reported issue remained.He replaced the pump pod and the issue was resolved.The unit operated to the manufacturer's specifications.
 
Event Description
It was reported that during set-up of the device for a cardiopulmonary bypass (cpb) procedure, the roller pump displayed a 'pump jam' message.As a result, an alternate device was employed.The surgical procedure was completed successfully.There was no delay, no blood loss, nor adverse consequences to the patient.
 
Manufacturer Narrative
During laboratory analysis, the product surveillance technician (pst) observed that when the pod subassembly was installed into a lab use only (luo) roller pump, pump jam errors occurred as soon as the pump was started, even without tubing or occlusion.He observed the resistor at r31 of the application board was loose.The solder termination had been broken and the unattached end of the resistor was floating above its attachment point.This would impact the output of the circuit which connects to the roller pump gut tachometer signal causing persistent pump jam messages.Once the resistor was repositioned to complete the circuit the pump jam errors stopped occurring.If additional information becomes available on this complaint that would alter the facts and/or conclusion, a supplemental report will be filed accordingly.
