It was reported this was a mitraclip procedure to treat degenerative mitral regurgitation (mr) with a grade of 4+, flail and pre-existing ruptured papillary muscle.One xtw clip was successfully implanted.To further reduce mr, an additional xtw clip was inserted and deployed on the mitral valve.However, after deployment, the second clip detached from the posterior leaflet and remained attached to the anterior leaflet (single leaflet device attachment/slda).To stabilize the slda, an additional clip was implanted, reducing mr to a grade of 3.There was no clinically significant delay in the procedure.

The device was not returned for analysis.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to the reported event.Additionally, a review of the complaint history did not indicate a lot-specific product issue.All available information was investigated, the reported slda associated with the clip detaching from the posterior leaflet appears to be due to patient conditions (anterior prolapse and flail extending medially and a lot of motion on the anterior leaflet due to the billowing prolapsing segment and ruptured papillary muscle).Unexpected medical intervention was a result of case-specific circumstances.There is no indication of a product issue with respect to manufacture, design or labeling.
