Event Description
Information was received from multiple sources (manufacturer representative, healthcare provider, clinical study) regarding a patient with clinical id (b)(4) and study id (b)(6) having spinal fusion therapy.It was reported that bilateral s1 screws loosening were noted on ct and x-ray scans.Levels treated: l2-s1, target levels: l4/l5, l5/s1.No hospitalization/prolongation of existing hospitalization; and no surgical treatment reported with this ae.Site assessment: not related to procedure/ plf grafting/ posterior fixation/ interbody fusion.Mdt assessment: not related to procedure/ plf grafting/ interbody fusion.Causal relationship to posterior fixation.There were no patient symptoms reported.There were no further complications reported regarding the event.Medical history (positive response): primary diagnostic indication all other diagnostic indications: stenosis with documented preoperative instability.Substance use- currently consumes alcohol and ex-use of tobacco.
 
Event Description
Additional information received that the loosening of the implant was caused by the fall and not because of a product.While this dose involved the surgical screws, but the cause of the ae was the fall.Investigator¿s assessment of not related to surgical construct and/or study procedure.Also, the sponsor has updated assessment to not related to construct/study procedure.
