Event Description
Medtronic received information that during use of a cardioblate gemini, it was reported that the customer noted that the device had broken at the tip after the first lot of ablations on the right hand side (convex and concave) and when maneuvering the device out of the chest to move to the left hand side.The issue occurred after the device had delivered 22 ablation lesions for a total of 15.42 minutes.The device was replaced to complete the procedure.A second device was opened and the left sided ablations were completed with no other issues.There was no adverse patient effect associated with this event.Medtronic received additional information that the jaws of the device did not clamp onto a surface besides tissue.The jaws were closed and locked 22 times.The broken tip did not fall into the patient as it was still attached to the guide.
