Event Description

During a procedure one onyx frontier drug eluting stent was implanted. The patient suffered from a st segment elevation myocardial infarction (stemi). The patient was readmitted with inferolateral stemi and an angiogram showed 100% thrombolytic occlusion in the lcx.Stent thrombosis was confirmed by the angiogram. The mi was classified as a spontaneous mi in the territory of the target vessel which was related to definite stent thrombosis.A percutaneous coronary intervention (pci) was attempted but was unsuccessful. The patient was given intravenous integrellin bolus and infusion with a low molecular weight heparin (lmwh) after.Three days later the event was treated with a pci to the lcx which was successful with a good angiographic result. The patient was not taking dual antiplatelet therapy within 24 hours prior to the event as the patient had run out of tablets. The patient recovered with sequelae. It was stated that the rationale for the relationship to system or procedure as per the investigator is due to the thrombolytic occlusion in the lumen of the recently implanted onyx frontier stent which cannot be ruled out as a factor.Approximately 3 weeks after the start of this ae, the patient underwent an index procedure during which the right coronary artery (rca) was treated with a euphora balloon catheter and prevail drug coated balloon and the circumflex artery was treated with 7 nc euphora balloons.

Additional information: during the index procedure a prevail dcb, 7 nc euphora balloons, 2 euphora balloons and one launcher guide catheter were used to treat lesions in the rca and cx.A non medtronic stent was also implanted during this procedure not an onyx frontier stent.Approximately 4 days post procedure the patient suffered from stent thrombosis and a mi.The site confirmed that the ae was not associated with a medtronic product. It was previously reported that an onyx frontier stent was used during the procedure. However new information was received which confirmed that a non medtronic stent was used not an onyx frontier stent. The thrombotic occlusion occurred in the lumen of the recently implanted non medtronic stent. The mi that occurred was related to this stent thrombosis. The event was assessed as not related to any medtronic devices.Medtronic is submitting this report to comply with fda reporting regulations under 21 cfr parts 4 and 803.This report is based upon information obtained by medtronic, which the company may not have been able to fully investigate or verify prior to the date the report was required by the fda.Medtronic has made reasonable efforts to obtain more complete information and has provided as much relevant information as is available to the company as of the submission date of this report. This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report. In particular, this report does not constitute an admission by anyone that the product described in this report has any ¿defects¿ or has ¿malfunctioned¿.These words are included in the fda 3500a form and are fixed items for selection created by the fda to categorize the type of event solely for the purpose of regulatory reporting.Medtronic objects to the use of these words and others like them because of the lack of definition and the connotations implied by these terms. This statement should be included with any information or report disclosed to the public under the freedom of information act.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.A good faith effort will be made to obtain the applicable information relevant to the report. If information is provided in the future, a supplemental report will be issued.
