Event Description
It was reported that shaft break occurred.During unpacking of a 2.00mm x 15mm emerge, resistance was experienced upon removing the device from its protective jacket, and the shaft of the balloon fractured into two parts which occurred 17cm from the center.A kinked was also noted on the shaft.The procedure was completed with another of the same device.There were no patient complications reported, and the patient status was stable.
