Event Description
An endurant limb (b)(6) was intended to be implanted during the treatment of a ruptured 55mm abdominal aortic aneurysm.It was reported during the index procedure, (b)(6) was inserted and aligned with the bifurcate flow dividerfor deployment.The graft cover was lowered but the stent graft also lowered together with the graft cover.The outer graft cover was deployed 3cm short of the stent sheath, but the stent graft remained attached to the outer graft cover, preventing its proper deployment.Due to this, it was deemed impossible to implant the stent graft, and it was consequently collected and removed.The stent was replaced with a non mdt device.Per the physician the cause of the deployment issue is undetermined.  no additional clinical sequalae were provided and the patient is fine.

Device evaluation summary: the device returned with the external slider partially open.The graft cover was partially retracted with five (5) stent rings exposed.The spital tubing was kinked with separation of the spirals visible proximal to the taper tip nose cone.The graft was deployed without issue.A kink was visible on the spiral.The reported deployment/expansion issue was confirmed through analysis.Medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report.This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
