Event Description
It was reported from japan that during a knee workshop procedure, it was discovered the battery oscillator device operation was switched on and off automatically due to vibration.According to the reporter the on/off switch was floating and when connection was tried, the section from the switch to the blade tip came off in one piece.It was not reported if there were any delays to the surgical procedure.It was not reported if a spare device was available for use.There was patient involvement reported.There were no reports of injuries, medical intervention or prolonged hospitalization.All available information has been disclosed.If additional information should become available, a supplemental medwatch report will be submitted accordingly.

This report is being submitted pursuant to the provisions of 21 cfr, part 803.This report may be based on information which has not been investigated or verified prior to the required reporting date.This report does not reflect a conclusion by depuy synthes, or its employees that the report constitutes an admission that the product, depuy synthes, or its employees caused or contributed to the potential event described in this report.Device was used for treatment, not diagnosis.If information is obtained that was not available for this medwatch, a supplemental medwatch report will be filed as appropriate.: device evaluation: the actual device was returned for evaluation.During repair, it was determined that the reported condition that the battery oscillator device operation was switched on and off automatically due to vibration and the on/off switch was floating was not confirmed.Therefore, an assignable root cause for the reported condition of loose, vibration and intermittent operation was not determined.However, during evaluation, it was determined that the handpiece was received in a disassembled state and the saw head was separated from the handpiece.Due to this failure other test steps failed and could not be performed however it was determined that the device failed pretest for general condition and check positioning of saw head.It was further determined that the electrical ports showed signs of corrosion and that the locking knob was not going into the teeth of the straining ring when the saw blade coupling was opened, but the saw blade coupling was closing normally.During the disassembly of the locking knob, it was found that the lock f/saw corner was rounded up which led to the failure.The assignable root cause was determined to be due to component failure from wear.
