Additional information was added to d9, h3, h4, h6, and h10: h4: the lot was manufactured between july 5, 2023 ¿ july 7, 2023.H10: the device was received for evaluation.A visual inspection was performed, and it was noted that there was fluid inside the bag that contained the unit.When the unit was removed from the bag, the cause of leak inside the bag was found to be an untightened red luer cap.The red luer cap is not a baxter product.The customer did not use the folfusor's blue winged luer cap; therefore, the cause of leak inside the bag was due to a use error.No evidence of rupture was observed from the bladder.The bladder was in normal condition.A functional leak test was performed, with no issues noted.Based on the sample analysis finding, the folfusor unit was determined to be conforming product.The reported condition of a ruptured bladder was not verified; however, a leak was discovered due to a user error.A batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.Should additional relevant information become available, a supplemental report will be submitted.
