Event Description
Medtronic received a report that a marathon catheter become occluded and had ruptured during the onyx injection.The reported device and accessory devices were prepared as indicated in the instruction for use (ifu).The patient was undergoing avm treatment.The access vessel was the venous access via the right arm (4 mm).The patient¿s vessel tortuosity was normal.Coil embolization had been completed and onyx was injected.The distal end of the marathon occluded and slightly proximal to tip of marathon ruptured and onyx leaked out.The marathon was removed and the patient had no issues post procedure.There was no friction or difficulty during delivery.The catheter was flushed as indicated in the ifu.The injection rate was 0.3 ml/minute.Aside from rupture, there was no other damage to the catheter.There were no patient symptoms or complications associated with this event.
