Event Description
According to the reporter, during an exploratory laparotomy small bowel resection, open procedure, the stapler was fired once successfully, second firing, the black firing knob snapped off unexpectedly and a small clear plastic piece fell into the patient.It took 5-10 minutes to find the black firing knob and small plastic piece and was retrieved.A new device was used to resolve the issue.
