Event Description
As reported, when testing this swan ganz catheter, the balloon did not deflate at all when attempting to deflate it.Replacing the device for another one, the issue was solved.There was no allegation of patient injury.The device was available for evaluation.
 
Manufacturer Narrative
The swan ganz catheter involved in this case was received by our product evaluation laboratory for a full evaluation.The balloon inflated clear and concentric and remained inflated for five minutes without leakage.The balloon deflated within three seconds without a syringe attached.Maximum deflation time from full capacity is four seconds per specifications.All through lumens were patent without any leakage or occlusion.No visible damage was observed on catheter or returned syringe.Complaint histories for all reported events are reviewed against trending control limits on a monthly basis, and any excursions above the control limits are assessed and documented as part of this monthly review.

Updated section h6 (component code) and h6 (investigations conclusions).Added information to section d4 (expiration date), h4 (device manufacturer date), h6 (type of investigation).The manufacturing records were reviewed for the lot involved and there is no indication of a related nonconformance.All process parameters were met and inspections passed successfully.Based on further engineering investigation, the root was was unable to be confirmed through product evaluation since the affected unit was returned for evaluation and no defect was found.
