Event Description
It was reported that the hub separated from an ultrathane mac-loc locking loop biliary drainage catheter.The catheter was inserted over a competitor's wire guide into the patient but was difficult to advance to the bile duct.At some point during the procedure, the hub separated.A new catheter was obtained to complete the procedure.As reported, the patient did not experience any adverse effects or require any additional procedures due to this occurrence.
