Event Description
Medtronic received information that prior to use of this transcatheter bioprosthetic valve, before opening the package, the actuator of the delivery catheter system (dcs) separated.The dcs was not used and was replaced.No adverse patient effects were reported.

Event Description
Additional information was received which reported that there was no damage to the packaging.
 
Manufacturer Narrative
Product analysis: upon receipt at medtronic¿s quality laboratory, the original medtronic box was returned with slight creases to the midsection and proximal end.There was a slight dent along the proximal end of the medtronic box.When removing the pouch and tray, the actuator and carriage were visibly detached.There was a dent to the pouch and a distal section of the pouch label damaged directly below the box dent and above the actuator and carriage.Both pouch seals were intact.The pouch was opened and removed, and there was no visible damage to the loading tray.There was a detachment to the proximal clip of one of the carriages, with white stress marks at the detachment site.The detached portion of the carriage clip was found at the midsection of the loading tray.There were white stress marks to the bases of both distal actuator clips.There was no damage visible to the screw gear and inner slider.The capsule was intact with no evidence of damage.The inner member shaft and spindle hub were intact with no evidence of damage.The r eported event for separation of components could be confirmed in the analysis.Updated: d9, h3, h6 medtronic submits this report to comply with fda regulations 21 cfr parts 4 and 803.Medtronic has made reasonable efforts to provide as much relevant information as is available to the company as of the submission date of this report.This report does not constitute an admission or a conclusion by fda, medtronic, or its employees that the device, medtronic, or its employee caused or contributed to the event described in the report.Any required fields that are unpopulated are blank because the information is currently unknown or unavailable.Medtronic will submit a supplemental report if additional relevant information becomes known.
