The device was returned and the evaluation found no reportable malfunctions.In speaking with the olympus technical assistance center (tac), it was reported that the contacts and the socket on the clv-190 were cleaned.Tac then requested the customer reseat the maj-1941 and maj-1933.After resetting, a b30 error occurred with no scope connected.Tac then asked the customer to reseat the 2 cables again, which resulted in getting no video.Should additional relevant information become available, a supplemental report will be submitted.
 
Event Description
It was reported, that the light source had a snowy distorted image and a b30 scope communication error.The issue occurred during preparation for use for an endoscopy procedure.There were no reports of patient harm.

The intended diagnostic procedure was completed with a similar device.
 
Manufacturer Narrative
This supplemental report is being submitted to provide the results of the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 11 years since the subject device was manufactured.Based on the results of the investigation, and since the device malfunction was not confirmed during evaluation, the definitive root cause of the reported issue could not be determined olympus will continue to monitor field performance for this device.
